You (the applicant) full understand that they are a consulting form, not a registered medical provider in any way. All recommendations are undertaken at your own risk. All information you obtain will be directly from their own experiences. If is of the best interest of anyone looking to start any bodybuilding or fitness program to first consult a physician before doing so, including a comprehensive  medical exam provided by a licensed  medical physician.

You (applicant) fully understand that you (applicant) may injure yourself as a result of participation in this program, and nearby release M Peramaki Fitness and Team Wilson from any liability now or in the future. For any injury including but not limited to: heart attack, stroke, broken bones, strained muscles, heat prostration, and any other illness or soreness caused, occurring during or after your participation in the fitness program offered.

By submitting this application you (applicant) state that in consideration of your participation in any fitness program or coaching you receive, you yourself, your personal representatives, administrators, heirs and assigns hereby holds harmless M Peramaki Fitness and Team Wilson from any claims arising from your participation in the program provided.

Buy submitting this application you affirm that you have read, have been honest with us about the information provided, and also fully understand the above information. As well as your responsibility to continue to keep your coach informed of current ongoing health and wellness information. You agree that you have been given the opportunity  to present any questions in all related matters.
